Description: The DAISY Foundation’s Health Equity Grants support nurse-led research and evidence-based practice projects that address the Social Determinants of Health and improve outcomes for underserved populations. Funded projects must focus on patient- and family-centered care and may address conditions affected by economic stability, education, healthcare access, environment, or social context. The Foundation has a particular interest in projects that promote healthy aging among older adults. Grants support direct project costs for nursing-driven solutions aligned with local community needs.
